Fill out and print this form and we’ll be looking for your repair on our next trip! *delivery is typically weekly, though times and days are subject to change. **DURING THE SUMMER MONTHS we do not make regular scheduled trips to the schools. Payment Options*
Pre-entering credit card information allows us to fix your instrument and deliver it our next trip. For any repair over $200: you will be contacted to complete the payment even when the card is pre-authorized. When pay by phone is selected, we will contact you after the service has been completed. *Be aware, we are unable to deliver the instrument until the repair is paid for, and if we are unable to contact you by phone prior to the next scheduled delivery date, delivery may be delayed*

Payment Autorization*
This payment method will be charged for the short term rental fee based on your instrument type. For any repair requiring over $200: you will be contacted to get authorization for the repair. The short term rental covers the time it takes for us to fix your instrument and have it back to the school. Instruments that are not available to be returned back may be subject to additional charges.

Current Rental Coverage + Loaner Option*
All of our current rental customer accounts have Maintenance & Repair Coverage. We also try to provide loaners for our rental accounts when we have the instrument availability. Since some of the school’s have loaners available for students, we like to ask if one is needed for you. *please keep any cleaning supplies, reeds, and lubricants, loaners will not have any accessories provided with them.
